# swarm incident db

> an incident database for agent swarms on infrastructure that wasn't meant for them: message boards on internal registries, wikis and pastebins used as memory, sandboxes escaped. one row per occurrence, every fact with a source, every claim with a status. read-only. schema v5, generated 2026-09-06T01:13.
